In the new age of health care, readmission penalties are just one of several major programs driving towards an end game of more appropriate, value-based care. To remain viable, community hospitals will need to engage continuum players (LTCHs, post acute venues, physician networks, and tertiary hospitals) as partners in the new age. As new payment programs are implemented, the implications will inevitably ripple through the settings surrounding community hospitals and radically change health care delivery. With this context, our panel will address:
  • What is the state of readmission penalties, “never events,” ACOs, bundling pilots, and Medical home pilots?
  • How will readmission penalties work? What is the net effect to community hospitals?
  • What strategies and techniques are hospitals using to measure, analyze and reduce readmissions?
  • What still needs to happen, both culturally and infrastructure-wise, to support readmission reductions across provider settings?
  • What affect might movements such as telemedicine, health coaches, geriatric care management, and the use of mid-levels for primary care have on avoidable readmissions?
  • What will be the impact of payment changes on general acute care census? What will be the impact on non-hospital providers?
